The Lost Policy Form should be completed by all policyowners if the original policy document cannot be located.  If you are surrendering your policy or your policy is maturing, the $20.00 lost policy fee is not required.  If you are requesting a new policy schedule the $20.00 lost policy fee is required.  Please contact us to discuss how the $20.00 lost policy fee can be paid.  If you are requesting a new policy schedule, AML/CFT identification will be required (please see AML information page for details).  Please note, all original completed forms and pen signed certified documents must be returned to Foundation Life by post to enable the request to be processed.  If you hold power of attorney for a policyowner and are completing any documentation on their behalf, please contact us to discuss possible further requirements.
