Certificate of Non-Revocation
The Certificate of Non-Revocation is required each time a Power of Attorney makes a change to a policy on behalf of a policyowner.
This is required to confirm that the Power of Attorney held has not been revoked.
Please note, Foundation Life also require a certified copy of the Power of Attorney document to enable a Power of Attorney to act on behalf of a policyowner, however once this document is held on file along with the Attorney’s AML/CFT identification (please see AML information), it is only a newly completed Certificate of Non-Revocation that is required each time the Attorney makes a change to a policy on behalf of a policyowner. Please note, all original completed forms must be returned to Foundation Life by post.
