Scheme Cash Payment Update

Foundation Life has completed over 13,000 cash payments totaling over $230 million and representing 86% of total Scheme cash payments to be made.
For those who have yet to provide the required cash payment documentation, we encourage you to act now. We have sent you two follow-up letters, and an e-mail to those for whom we have an e-mail address.
Foundation Life is in the process of winding up the company. If we do not receive your documentation promptly, in due course, your payment will be transferred to the Inland Revenue Department (IRD) in accordance with the Unclaimed Money Act 1971. You would then need to claim your cash payment directly from the IRD. We will send a final notice letter before this occurs.
To receive your cash payment, please:
• complete the cash payment form previously provided; and
• provide the supporting documentation outlined in our earlier correspondence.
